GUERRILLA FOOTBALL (GF) WAIVER OF LIABILITY AGREEMENT 

This waiver is effective from the date it is signed until December 31, 2026. It covers workouts, practices, team building events, training camps, and all other training camp related activities.

Disclaimer

The organizers are not responsible for any injury, loss, damage, death or any claims, demands, actions and cost as of any kind suffered by the participant while participating in the camp. I understand “Organizers” to mean: Guerrilla Football, Guerrilla Football coaches, owners/operators of the practice facility/field and each of their respective directors, officers, employees, coaches, volunteers, and members.

Description of Risks

I acknowledge that I am aware of the possible RISKS, DANGERS, and HAZARDS associated with my
participation in training activities, INCLUDING THE POSSIBLE RISK OF SEVERE INJURY TO PARTICIPANTS
OR OTHERS. These risks include, but are not limited to:
• Muscular injuries resulting from vigorous physical exertion and strenuous cardiovascular workouts
including rapid movement, quick turns and stops.
• Bruises, sprains, cuts, scrapes, broken bones and dislocations resulting from contact and collisions with other players, goal posts, and the ground.
• Injuries to the eyes, teeth, face, head, and other parts of the body resulting from contact and collisions with other players, goal posts, and the ground.
• Neck and Spinal cord injuries, which may render me permanently paralyzed or result in my death.
• Participating against older, larger and more experienced players.
• I agree to wear a mouth guard and understand that if I choose not to wear a mouth guard that I am exposed to a much greater risk of injury.

Photo-Video Release

I hereby authorize GF to use pictures of me (or my child/ward) taken in a photograph, digital image,
videotape, motion picture, and/or testimonial (written words). The undersigned hereby releases GF, its agents or employees, as well as any and all users and exhibitors of said pictures, from any and all claims, demands, accountings, and causes for which the aforesaid videotape, testimonial, motion picture, digital image, or photograph likeness may be used pursuant to this Consent and General Release. It is also my understanding that I will receive no compensation for my likeness or testimonial.

Release of Liability

In consideration of the Organizers allowing me to participate in training related activities, I agree:
• To assume all the risks arising out of, associated with or related to my participation in these activities, even though such risks may have been caused by negligence of the Organizers.
• To be solely responsible for any injury, loss, or damage that may have been caused by the negligence of the Organizers.
• To release the Organizers from liability for any and all claims, demands, actions, and costs that may arise out of my participation in any and all team-related activities, even though such claims, demands, actions, and costs may have been caused by the negligence of the Organizers.

Use of Data

GF shall have the right to use the participants performance data collected during training and
benchmarking sessions to help create, develop, operate, deliver, and improve its products, services,
content and advertising, and for loss prevention and anti-fraud purposes.
